Why he might be annoying:

    Given that he had only one brain (and thus presumably one personality), was it appropriate to give him two names?
    Only one of his two mouths was functional as the other mouth was missing its lower jaw.
    His shared middle eye was nonfunctional and did not blink, making him look like he was staring, even when the other eyes were closed.
    His owner said about people's reactions when they came close to pet him, 'I see a look of horror come over their faces when they clearly see his faces. It's funny.'

Why he might not be annoying:

    Most Janus cats die within days of birth, making his longevity remarkable.
    One veterinarian suspected the secret to his survival was that he developed only one esophagus, which connected to the one working mouth.
    He liked to be walked on a leash.
    His owner described him as 'very laid back, not afraid of people, very friendly.'
